To receive your GSA Committee Service Stipend, please follow the following steps to submit your required GSA Committee Report, due on or before the last day of the quarter following your term of service:

1. Click on this link: Blank Committee Report Form and a new window containing an editable PDF will appear.
2. Using the menubar that appears at the top of the screen, select SAVE AS and save to your documents.
3. Open the new document, complete, and either Print or Save.
4. Email your completed form to the VP of Committees & Planning, or deliver a hard-copy of the completed form to the GSA Lounge front desk Staff (route to Dusty Hoesly, VPCP).

You should receive payment within three weeks of submission. Thank you for your service!
